- Use the OCD form on http://mdiwww.nascom.nasa.gov/socd/soho_ocd_form.html
  (there is a link to it from the Observers Manual & Planning Information
   section of the Operations Manual)
  
- Fill out forms located in the Change Directorives Binder in the MDI room
  in the EOF.  Sample forms are also located there in addition to blank forms.
  Submit the forms to the FOT.
